Each year the Duxbury Board of Health enters into a contract with Partners Home Care.
Partners provides:
Communicable Disease Control including follow-up on cases of reportable communicable and/or infectious diseases.
Home visits to high risk maternity patients providing pre and post-natal health supervision, instruction, assessment of the newborn/premature infants, and children with congenital defects.
Home visits to residents who have no health insurance or whose health insurance does not pay for home visits, to perform assessments, evaluations for services, specific treatments and cooperation with other services.
Partners holds weekly clinics at the Duxbury Senior Center to provide health counseling, blood pressure screening, and immunization.
Clinics for adults and children are held at Partners Home care at 38 Resnik Road, Suite 103, Plymouth, on Tuesdays and Thursdays from 1:30 p.m. till 3:30 p.m. Adult clinics offer immunization, and TB (Mantoux) testing. TB testing will be done on Tuesdays and readings done on Thursdays.
Each fall Partners Home Care provides the nurses for the Towns flu clinics.
